Welcome to the registration process! Upon completion, you will create a user account which you may use to login to the system in the future. If you have already registered, you may wish to login or recover your login instead.

To register, we need some information about you. Please fill out the following form and select Continue.

Employees and Spouses must each create their own separate and unique account
Employees should use the information as it exists in Workday
- If you are unsure of how your information exists in Workday you can contact AskHR@medtronic.com
- If both you and your Spouse are employed with Medtronic, please use your own Employee ID, First Name, Last Name, and Date of Birth to create your account
Spouses (who are not Medtronic employees) but that are on the Medtronic Medical Plan should use their own First Name, Last Name, and Date of Birth.
- IMPORTANT – When filling in the Employee ID field, you must use the insured employee’s Employee ID followed by an "s"
Spouses who are not on the Medtronic Medical Plan must call Empower Health Services at 866-367-6974 to register
